Fixed prostheses are made of two distinct parts: an internal core that provides strength and a veneering layer that provides aesthetics. The ideal material for fabricating bridges will depend on several factors that must be evaluated by the professional when helping the patient choose. Factors to consider include the location of the bridge, the function of the missing tooth or teeth, aesthetic characteristics and importance, and the cost.
The core, which is stronger, can be fabricated from metal alloys, such as Cobalt-Chromium and Nickel-Chromium. It can also be made with precious metal alloys, such as palladium-silver or titanium, which are indicated for patients with metal allergies. Another option for cases with higher aesthetic demands is alumina or zirconia; being light-colored, they allow for a highly aesthetic appearance.
The veneering layer of the prosthesis is made of ceramic or porcelain. It is of mineral origin and, with proper handling and bonding, delivers a very natural aesthetic finish. The final aesthetics of this type of prosthesis heavily depend on the ceramist, who adds the final artistic touch to the prosthesis in the dental laboratory. There are two modalities of fixed prostheses on teeth, depending on whether they replace missing teeth or solely restore damaged teeth: crowns and bridges.
Types of fixed prostheses on natural teeth
Dental crowns
Crowns, also known as dental caps, are ideal for restoring individual teeth damaged by fractures or decay. They completely cover a tooth, restoring its proper shape and appearance. Furthermore, crowns are also used to strengthen a weakened tooth and reduce the risk of fracture, cover a discolored or misshapen tooth, and reinforce a tooth that has undergone root canal treatment, among other indications.
To place these crowns, the natural tooth must be prepared or trimmed to make room for the ‘cap’ so that it fits perfectly over the prepared tooth (known as the ‘abutment’).
This type of dental prosthesis is handcrafted by a dental technician in a laboratory using models of the patient obtained after the tooth preparation. Matching and adapting the crown to the color, spacing, and shape will be key to achieving the highest possible blending of the prosthesis with the rest of the mouth.
As previously explained, these can be porcelain-fused-to-metal or all-ceramic when the core is made of alumina or zirconia.
Dental bridges
Dental bridges are used to replace at least one missing tooth. Since it attaches to the two teeth adjacent to the missing piece, the bridge consists of a minimum of three units. It is possible to create bridges with more units, although the dentist must indicate when the forces might be excessive for the success of the treatment.
The two teeth located at the ends of the bridge are called abutments due to their supporting function, and the units of the bridge attached over those teeth are the retainers. It is worth noting that the abutment teeth are prepared beforehand to allow for the cementing of the retainers. The central unit that replaces the lost tooth is called a pontic.
The most common materials for bridges are metal and ceramic. In the aesthetic zone, it is possible to use zirconia again as a core, but using alumina is not recommended as it has insufficient strength in these cases.
Being fixed, this type of bridge provides comfort, confidence, retention, and stability; however, the importance of hygiene and periodic visits to the dentist must not be neglected to extend its lifespan.
