What are fixed prostheses on dental implants?

As the name suggests, fixed prostheses are attached to the implants and are the closest option to the lost natural teeth, since they do not move and cannot be removed by the patient.

This type of prosthesis is made of two types of materials: a core material providing strength—which can be metal or high-strength ceramic—and a veneering layer of feldspathic ceramics that provides aesthetics and perfectly mimics natural teeth due to its qualities.

Cement-retained vs. screw-retained prostheses?

Fixed prostheses can be either cement-retained or screw-retained to the implants. In cement-retained cases, an abutment is screwed onto the implant first, and the prosthesis then encompasses this abutment and is bonded to it using a dental cement.

In many circumstances, both cementing and screwing are valid options that achieve equally successful results. However, there are exclusive indications for each type that the professional must identify before fabricating the prosthesis. For instance, if there is limited space up to the opposing teeth (the upper teeth if we are working on the lower arch, or vice versa), a screw-retained prosthesis is indicated because a cement-retained one would have insufficient retention. Conversely, in the aesthetic zone, the implant’s direction may sometimes make a cement-retained prosthesis essential to prevent the screw access hole from emerging through the outer surface of the tooth.

Hybrid prostheses

Hybrid prostheses are a specific type of fixed prostheses made of metal and acrylic resin, and they are always screw-retained. This type of prosthesis is only fabricated for completely edentulous cases to restore a full maxillary or mandibular dental arch. To achieve this, between 4 and 6 implants are placed per arch.

These prostheses feature a metal framework that provides strength, covered by pink acrylic resin which, in turn, supports artificial resin teeth. Currently, the metal framework is computer-designed and computer-manufactured (CAD/CAM), achieving high precision. Furthermore, modern resins allow for aesthetics that are virtually on par with ceramics.

This type of prosthetic rehabilitation is indicated when the patient has not only lost their teeth but has also lost, or is expected to lose, part of the bone and gum tissue. The pink resin over the metal framework allows for the replacement of missing bone and gum, in addition to the teeth. Thus, it restores the patient’s self-confidence through excellent aesthetics, functionality, and comfort. It is the ideal option for full-arch fixed rehabilitation on implants for patients with limited available bone.

Step-by-step process for fixed prostheses on implants

For the placement of fixed prostheses—whether hybrid or screw-retained—the patient’s implants must be properly osseointegrated. In other words, they must have previously undergone the surgery to insert the titanium implants into the jawbone, which are what will support the prostheses.

In subsequent visits, records will be taken to gather the information needed to first fabricate the supporting metal framework, followed by framework try-ins and aesthetic evaluations. This requires anywhere from 1 or 2 sessions in the simplest cases, up to 3 or 4 sessions for aesthetic zones or multiple-teeth restorations.

Once the prosthesis is completed, it is either screwed onto the implants or cemented after the intermediate abutments have been screwed into place. The final step for screw-retained prostheses is sealing the screw access holes with composite resin, a procedure that closely resembles a simple dental filling.
